Online Tutoring Evaluation

In order to provide the highest quality tutoring service, we need to periodically evaluate our program. The purpose of this form is to give you an opportunity to evaluate the tutoring assistance you have received. Your honest response will be appreciated.

Please complete one evaluation for each Tutor or Learning Assistant from whom you received assistance. Thank You!

For questions with responses based on a 1-5 scale; 5 is meant as the most favorable response (i.e., excellent or usually), while 1 is meant for the most unfavorable response (i.e., unsatisfactory or rarely).
