This form allows Recognized Student Organizations to request academic classroom spaces at the University of Southern California. Please allow up to four (4) weeks for confirmation of requests made at the start of each semester.

NO AMPLIFIED SOUND is permitted in classrooms; auditoriums only. Rooms marked with two asterisks (**) above are locations that permit (but do not guarantee) amplified sound. If academic classes are scheduled in the immediate area of a request, the request will be denied if adequate space cannot be found.

INSTRUCTIONS FOR MULTIMEDIA SUPPORT REQUESTS/RENTALS
  1. Multimedia support can only be requested after you have received a confirmation of your room reservation and location.
  2. A detailed list of the equipment available in each classroom is located here: http://www.usc.edu/its/multimedia/auditoria/matrix.html. You will only be able to request equipment that is available in the room you have been assigned.
  3. Multimedia support must be requested via the Special Event Equipment Request Form available at http://its-mysql1.usc.edu/av-requests/single_request_b.php
  4. You must also submit a Service Level Agreement (SLA) Form each year. One SLA will cover all of your classroom reservations for the school year. If you do not already have an SLA on file with ITS, you can fill one out here: http://www.usc.edu/its/multimedia/reservation/agreement.html
  5. Requests must be made at least 1 week in advance of your event date. Late requests will not be accommodated.
  6. Some multimedia support services incur rental or staffing charges. Events requiring multimedia support after 9:30pm, on weekends or on holidays will always incur staffing charges.

Classroom requests take approximately five to seven business days to process. During peak times this process could take longer. Academic classes are given priority in classroom assignments. Food and drinks are not permitted in classrooms or auditoriums. Complaints on disturbing academic classes may result in cancellation of all current and future reservations. Fees are charged accordingly.
Academic classes have first priority to all classroom space. Your organization's classroom assignments may be relocated. If this occurs you will be given at least 7 days of notice and an alternate location.
The University of Southern California reserves the right to cancel any event, in whole or in part if fees are not paid 2 weeks prior to the event, if the group does not comply with USC policies and procedures, if information for the event is altered without notifying the USC Scheduling office, or if USC officials cannot ensure the safety of students, faculty, staff or visitors.

This form serves as a request for space only. Events are not confirmed until you receive email confirmation from the Scheduling Office. By submitting this form, you are agreeing that your organization will comply with all USC policies and procedures.
