Intramurals Entry Form

• Registration may be online (see above) or in person at the Lyon Center using the Print Forms above.
• Team captains are encouraged to register online.
• Payment and registration must be submitted to the RecSports Office (Lyon Center) before the entry deadline to be placed in the league or tournament. Participants' signature may be done in person at the Lyon Center or at the first scheduled competition.
• Captains and individual participants are encouraged to print a copy their roster(s). Captains and Free Agents are required to attend the captains meeting of their respective sport activity.

TEAM REGISTRATION
(Registration for team sports such as flag football, soccer, volleyball, floor hockey, 5-on-5 basketball, etc.)

1. Managers (team captains) complete the IM entry form (available at the Lyon Center and online). Team members must complete and sign the entry form prior to their first contest at the field/court.
2. Managers/Captains shall complete a "captain's profile" and will be able to use this same profile to enter other teams and/or enter the same team in other sports.
3. IM entry forms and entry fees must be submitted to USC RecSports by the deadline date. Cash, check or credit cards are accepted. Make checks payable to USC Recreational Sports.
4. Managers (team captains) are required to attend managers meetings and/or schedule pick-up dates. Not all sports have scheduled managers meetings. Notification and confirmation via email and/or phone.

INDIVIDUAL (FREE AGENT) REGISTRATION
(Registration for individuals not affiliated with a full-team or want to be placed on a team.)

1. Players without a designated team can register as a FREE AGENT online.
2. Once FREE AGENT information is submitted, information will appear on the FREE AGENT list.
3. Teams looking for players can “pick up” a player from this list or players form their own FREE AGENT team. FREE AGENTS are encouraged to attend managers meeting.

REGISTRATION FOR NON-TEAM SPORTS
(Registration for individual sports such as tennis, squash, badminton, swimming, table tennis, golf, 3-point shootout, hot shots, fun runs, and racquetball.)

1. Participants must complete the IM entry form (available at the Lyon Center and on-line).
2. IM entry forms and entry fees must be submitted to USC RecSports by the deadline date. Cash, check or credit cards are accepted. Make checks payable to USC Recreational Sports.
3. Schedules are available at the RecSports Department prior to the contest (tournament, league or meet). Notification and confirmation via email and/or phone.
