Your sponsoring academic department should be sending you a two-page Certificate of Eligibility, Form DS-2019, which you are to present with your passport to a U.S. Consulate or Embassy to obtain your visa. Carefully read both pages of the DS-2019 form before you sign the bottom of page one.
