For HSC J-1 facilitators

J-1 visa services are available on the Health Sciences Campus to better serve departments and institutes such as the Keck School of Medicine, School of Pharmacy, Doheny Eye Institute, Zilkha Neurogenetic Institute, and Children’s Hospital Los Angeles.

While most J-1 procedures are identical on the UPC and HSC campuses, please note a few minor differences for HSC J-1 Facilitators listed below: 

APPOINTMENT LETTER 

This office requires an appointment letter, addressed to the Dean, with a signature of the PI, Division Chief and Department Chairman, in addition to the request for DS-2019 form.

OIS PROGRAM FEE

We prefer USC internal requisitions; however, in some cases a Departmental check is acceptable.

APPLICANT’S ADDRESS

All initial DS-2019 requests need to be accompanied by the applicant’s mailing address in the foreign country.

PASSPORT

Including a copy of the passport page with the applicant’s name helps us with the correct name spelling and sequence.
